The information you provide in this application will help us to ensure you and your future cat are a perfect match. Please be honest and descriptive in your responses.
*we ask that you keep your kitten strictly indoors until at least one (1) year of age
Cats allowed outside are exposed to dog attacks, cat fights, diseases, and unexpected circumstances. This may result in extensive veterinary bills. We highly recommend purchasing pet insurance for cats of all lifestyles.
The Pacific Cat Clinic strives to place kittens in the perfect home for their individual personalities and needs. To help us with this process we ask that you provide us with 3 references. Each reference will be called prior to approval of adoption and each will be asked a set list of questions. To accelerate this process, please advise your references ahead of time that we may be contacting them.
Please provide at least 2 personal references and 1 professional reference of someone you have known for at least 2 years and is not related to you.
If you are approved for adoption AFTER the references have been called we will be in contact with you and proceed to collect a deposit of $100.00 + tax to hold the kitten until pickup. This is included in the total adoption fee.
Every effort is made to ensure that any cat adopted from Pacific Cat Clinic is in good health. Preventative measures that have already taken place to ensure good health include a blood test for Felv and FIV, fungal culture and physical exams. The animals are, however, rescue cats and thus may have underlying health conditions that cannot be foreseen. Diarrhea and vomiting in kittens is not uncommon, especially in rescue kittens. Your kitten may show signs of vomiting/diarrhea when first brought home, this is usually a stress response. We have examined the kitten and found it healthy to the best of our knowledge at the time of discharge.
An apparently healthy animal may appear well at time of adoption, but may be incubating an infectious disease or parasite. Illnesses may be non-diagnosable. Pacific Cat Clinic reserves the right to charge for diagnosis, treatment, and management of this cat in the unlikely event that an unexpected medical problem develops. In the event that illness occurs within the first year of the kitten’s life, you as the owner, are responsible to cover the costs associated with the illness.
Pacific Cat Clinic is not responsible for the kitten’s medical needs in the first year of life outside of routine care. A six week free trial of Pet Health Insurance is also included in the kitten adoption program. This is STRONGLY RECOMMENDED to be carried on throughout the kitten’s life, as veterinary care (especially emergencies) can be quite costly at times. We encourage you to investigate insurance companies and policies over the next six weeks. The kitten will also be sent home with a goody bag, with some food to assist them in settling into their new home.
A discharge appointment will be made within 1 week of adoption approval, failure to uphold this appointment will forfeit your kitten along with your $100.00 deposit.
I have read the terms and conditions of the adoption agreement contract. I understand that in adopting this kitten from the Pacific Cat Clinic Adoption Program I am contracting to the following:
- Routine health and wellness visits in the first 4-5 months of life will be provided by the Pacific Cat Clinic. This will include vaccinations, surgical sterilization, and permanent identification with both a tattoo and microchip. This does not include the one-year wellness visit.
- I agree that I will ensure that my kitten is spayed or neutered before 6 months of age.
- I am financially able to care for the kitten and I understand that I must provide food, water and veterinary care. I understand that owning a cat can be costly at times, I will consider pet health insurance to help cover future unexpected costs.
- I will pay all the fees due for this adoption as required, and understand that the kitten may be confiscated with no refund of fees if I fail to fulfill my obligations to the Pacific Cat Clinic adoption program.
DEPOSIT FEE: In order to secure a particular kitten, we require a deposit fee of $100.00. This fee is non-refundable and non-transferable. Once your fee has been received, your application has been approved, and your references have been checked, we will hold that kitten for you until its final exam has been completed.
BALANCE OF ADOPTION FEES: At the time of pickup, you will be required to pay the balance of the fees due, which is non-refundable and totals $1,241.26 (including tax) for kittens and $450.74 for adult cats (including tax.)
You can arrange to do this over a payment plan of 4 months, provided a credit card is used. Payment in full at time of discharge is also an acceptable method of payment.
